Historically, man has squatted in order to defecate, and the practice continues today in the underdeveloped countries. The widespread use of a sitting toilet in the Western world began during the 19th century after the water toilet bowl and the sewage system was successful developed (1). The main part of the toilet bowl is the device in the form of letter "s" which together with bolus of water prevents the entry of gases from the sewage system into the houses. The "s" letter device explains the height of the toilet bowl. The developers of the toilet bowl didn't evaluate seriously the influence of the sitting toilet on the human health. The old and week and obese and handicapped were of cause happy with the sitting toilet bowl. But as early as the beginning of 20th century Hurst explained the constipation as a consequence of defection in the sitting posture (2), nearly 50 years later Bockus pointed out that the ideal position for defection is the squatting (3). The expression for difficulties with defecation in the sitting posture may be found in the numerous Patents for squatting toilet devices, registered beginning already from the end of 19th century and onward.
A group of physicians working in the rural Africa in the midst of 20th century found that several medical problems such as hemorrhoids, constipation, diverticulosis coli, and hiatus hernia are of very low prevalence in the local population, later they found that it is also characteristic for the other countries of the Third word.
In 1966 Tagart showed that that the rectoanal angle straightens with fully flexed hips-corresponding to the squatting position assumed for defecation-and converts the rectoanal outlet into straight canal, thereby facilitating rectal emptying( 4 ). In 1987 Sikirov showed in own control trial that the hemorrhoids may be cured by switching from the sitting to the squatting posture (5). In 1989 this author showed that the sitting posture obligate more intensive and pronged straining to empty the rectum than the squatting posture (6). In 2003 Sikirov by comparing two sitting posture of different heights with the squatting posture showed that rectal emptying in the sitting postures obligate 2-2,5 more straining than in squatting posture( 7) . In 2006 a group of 12 primary care physician in a controlled trial showed again that hemorrhoids can be cured by switching from the sitting to the squatting posture (published on this site).
            Self destruction nature of the sitting defecation posture cannot be expressed only in regard of the hemorrhoid tissue. Several other systems must suffer damage, however because of the methodological problems it cannot be confirmed directly as in case of hemorrhoids. But physiological, pathophysiological and epidemiological data point out that at least several problems such as constipation, diverticulosis coli, hiatus hernia are the consequence of defecation in the sitting posture.
            Practical continuation to the described data was the designing of a squatting device tailored for the Western world sewage system (see the page: video-film of a new squatting device). Switching to the squatting defecation posture will improve the health of the Western world people preventing unnecessary morbidity and mortality.
